МКФ-ориентированная самооценка функционирования после тотального эндопротезирования тазобедренного сустава: Разработка и валидация краткой шкалы

Цель исследования: разработать и провести первичную психометрическую валидацию краткого опросника самооценки функционального статуса на основе Международной классификации функционирования для применения в реабилитации после тотального эндопротезирования тазобедренного сустава.
Методы. Проспективное продольное исследование с повторными измерениями (n=158) проводилось в г. Астана (Казахстан) с октября 2025 по апрель 2026 года в трёх временных точках: первая – 2–3-й день после операции, вторая – 14-й день (выписка), третья – 3 месяца после операции. Содержательная валидность оценивалась экспертной панелью из семи реабилитологов. Психометрические свойства оценивались в соответствии со стандартами COSMIN: внутренняя согласованность (коэффициент альфа Кронбаха), ретестовая надёжность (внутриклассовый коэффициент корреляции), конвергентная валидность (коэффициент корреляции Спирмена с шкалой Харриса), чувствительность к изменениям (стандартизованное среднее разности), а также эффекты «пола» и «потолка».
Результаты. Содержательная валидность подтверждена (индекс содержательной валидности для каждого пункта не ниже 0.78; шкальный индекс не ниже 0.80). Внутренняя согласованность: альфа Кронбаха = 0.980 (n=152). Конвергентная валидность подтверждена сильной отрицательной корреляцией с шкалой Харриса: коэффициент Спирмена = −0.832 в первой точке и −0.842 во второй точке (р менее 0.001). Ретестовая надежность в модели согласования достигла внутриклассового коэффициента корреляции не ниже 0.70 для 14 из 15 пунктов. Стандартизованное среднее разности суммарного балла составило 6.57. Эффекты «пола» и «потолка» отсутствовали (0%).
Выводы. Опросник самооценки на основе Международной классификации функционирования после тотального эндопротезирования тазобедренного сустава является психометрически состоятельным инструментом с высокой чувствительностью к изменениям и пригоден для цифрового мониторинга функционального восстановления пациентов.
